The Biden-Harris Administration’s vision is to allow every American to live a full and healthy life in every community. They have created a playbook that sets the stage for agencies and organizations to re-imagine new policies and actions around social determinants of health (SDOH). - The White House (November 2023).

Community Care Hubs (CCHs) are community-centered entities that organize and support networks of community-based organizations (CBOs) to meaningfully and efficiently integrate CBOs and the health-related services they provide into the healthcare continuum.

About Us

Community Assistance and Transition Care of Houston (CATCH) has been a cornerstone of support for the Greater Houston area since 2006, formally establishing itself as a Community Care Hub (CCH) in 2023. Focused on addressing social determinants of health, CATCH supports older adults, individuals with disabilities, veterans, and caregivers through a collaborative network of public health organizations and community-based
entities.

Our work ensures efficient access to nutrition, transportation, and care coordination services, ultimately empowering clients to lead healthier, more self-sufficient lives.

Our Team

CATCH - Director, Janice Sparks, PhD

Janice Sparks, PhD

CATCH Director

Janice Sparks is a public and population health professional and gerontologist with a passion for improving the well-being of individuals and communities. She has extensive experience in program development, health equity, and addressing the needs of older adults and people with disabilities. Throughout her career, she has successfully translated policy into practice, strengthened health systems, and developed initiatives that promote population and individual health.

She is skilled in building teams, drafting strategic plans, implementing quality improvement protocols, and fostering partnerships. Her expertise has led to significant achievements, such as reducing hospital readmission rates, creating successful veteran services programs, and securing substantial grant funding.

She is a certified Lean Six Sigma Yellow Belt and HFMA Certified Revenue Cycle Representative, currently pursuing CPHQ certification. Her commitment to community health is evident through her active involvement in organizations such as the Houston Health Department Quality Improvement Committee and her past role as the state of Texas Aging and Disability Resource Center state co-Chair.
